Use one each of these forms to tell the judges about your model(s).  Make sure to fold the section with your name under the form so that only the model, not the builder, figures in the judging.  Your Entry Number from your Registration Form should appear on this Entry Form.  You can download this form and use the "Fill and Sign" feature to type your information into the form before you print it out.
